EMS application

These are a series of Questions that are for us getting to know who you are and what your skills and capabilities are.

1. Do you have any criminal records, What are the crimes you have committed?

2. Do you have any past experience with any first aid or any medical treatment knowledge?

3. Why do you have an interest within the EMS team? / Why would you like to join?

4. As a member of the EMS team, What can you bring to the team? / How can you help?

5. Do you have a valid drivers license?

6. What is your over all goal within the EMS team?

7. How much time are you willing to give to the EMS?

Note: You must have more than 10 active hours on the FiveM Server to apply.
